The Welcome Home Program supports people recovering from a hospital stay

People recovering from a hospital stay often come home to an empty refrigerator, and can't stay on their feet long enough to cook a nourishing meal.

The Welcome Home Program supports people recovering from a hospital stay with a care package of 12 weeks of nutritious, tasty meals within 48 hours of referral.

Our meals and other support services prevent future stays at the hospital and assisted living facilities so that healing and health can happen at home without the burden of more expensive care options.

How the program works:

  • Home delivery of 12 weeks of medically-tailored meals within three days of leaving the hospital
    Wholesome, homemade meals are prepared in our own kitchen and frozen for convenient reheating at a later time. All meals meet nutrition standards based on recommendations from the USDA’s Dietary Guidelines for Americans and the American Heart Association. Specialized diets such as vegetarian, no gluten/no dairy, or easy digestion are available. Any of our menu options can be made suitable for people with chewing and swallowing difficulties.

  • Also available: free sessions with a Registered Dietitian Nutritionist (RDN) to help you with your home recovery
    Our knowledgeable and caring RDNs can help with things like:

    • How to eat for your health conditions, such as diabetes, high blood pressure, cholesterol, and many others

    • Where to go for free or low-cost food

    • How to use food to help with symptoms like diarrhea, constipation, nausea, weight management, and more

ELIGIBILITY:

  • Must be discharged from inpatient care within two weeks of signing up for our services and be referred by a medical provider (could include a social worker, hospitalist, doctor, or other agency support representative).

  • Live in King County and be low-income. If you live outside of these boundaries, you may be able to get meals through our healthcare partnerships.

HOW TO ENROLL:
