Assessment & Consultation with a Registered Dietitian Nutritionist

Dietary counseling, often called Medical Nutritional Therapy, includes an assessment of your nutritional health and individual dietary needs. Our knowledgeable Registered Dietitian Nutritionists (RDNs) work one-on-one with clients, looking at things such as food intolerances, allergies, accessibility to healthy food, diet, medical test results, and mental health to support them in achieving them.

ELIGIBILITY:

We provide nutrition counseling at no cost for clients enrolled in Lifelong services living with:

  • Pre-diabetes and diabetes

  • High cholesterol

  • High blood pressure

  • Congestive heart failure

  • Kidney disease

  • Gastrointestinal (GI) issues such as IBS, Crohn’s, and ulcerative colitis, constipation, acid reflux, and other GI symptoms

  • Unintentional weight loss or weight gain

  • Chewing and swallowing difficulties

  • Food allergies and sensitivities

  • Other conditions which can be supported with nutrition
