What We Do
Lifelong is a community health organization working to make health and food accessible to all, no exceptions. People don't experience life's challenges one at a time which is why Lifelong provide holistic, wraparound care that includes healthcare access, housing, nutrition, and aging and disability services.
By showing up for our community every day, Lifelong is meeting immediate needs and building toward a future where everyone has the opportunity to live a healthy, joyful life.
Aging & Disability
We connect older adults, people with disabilities, and their caregivers access in-home care, navigate complex care systems, and connect to the resources they need to live independently.
HIV Support
We help people living with HIV stay connected to life-saving care through case management, insurance enrollment, medication assistance, dental services, and supportive resources.
Food & Nutrition
Through the Chicken Soup Brigade, we provide medically tailored meals, nutritious groceries, and nutrition counseling that support health and well-being.
Housing Services
We connect individuals experiencing housing instability to housing support, case management, and resources that create a stronger foundation for health.
