Long-Term Care
Medicaid Case Management provides an array of long-term services and supports. The primary goal of case management is to enable vulnerable adult to reside in the setting of their choice with services and supports that maximize independence, dignity, and quality of life.
Food & Nutrition
Lifelong’s Food & Nutrition program, Chicken Soup Brigade, delivers healthy, home-cooked meals and nutritious groceries right to your door. Meals are made on-site, from scratch, using whole food ingredients. We support people’s overall health by focusing on the importance of nutrition as part of someone’s care. Food and nutrition can reduce the symptoms and daily impact of many illnesses, minimize medication side effects, and provide comfort when it is needed most.
Dental
The Lifelong Dental Program provides comprehensive oral health access for low income individuals living with HIV who reside in King, Island, and Snohomish Counties.
Good oral health is essential if you’re living with HIV. Soreness, pain, and missing teeth can keep you from eating the foods you need to stay healthy and can lead to problems in other parts of your body.
Housing Services
Need help navigating the complicated world of housing assistance? We’re here for you. Lifelong manages nearly two dozen units in North Seattle for people with severe medical conditions and 160 units throughout Seattle for anyone over the age of 24.
Depending on your unique circumstances, we offer three different housing options:

The Health Insurance Program provides essential support by paying insurance premiums for low-income individuals living with HIV in the state of Washington. Access to medications, doctor's appointments, and necessary care is vital.
