About the Hub

Navigating the complex systems to find care and support services for individuals with disabilities can be difficult.

The East King County System Navigation Hub connects caregivers and community members with its broad network of provider organizations, removing bureaucratic barriers to quality care and essential training and services.


Need Suppport Services?

We partner with a collaborative network of providers and organizations with the goal of helping community members establish holistic, wraparound care teams that meet their individualized needs.

Many of our partners also provide vital training courses for both client and caregiver education and enrichment.

We connect people to:

  • Food, Meals, Transportation: Food Bank, Grocery Delivery, Medical Transportation

  • Health + Medical: Peer Mentorship, Health Care Coordination, Advance Care Planning

  • Advocacy: Legal, LGBTQ+, Disabilities Rights Advocacy

  • Case Management: Traumatic Brain Injury, Interpreting and Translation, Family

  • Housing: Minor House Modification and Repair, Dementia and Memory Care, Moving Assistance


Interested in becoming a Partner Organization?

Join us as we come together to:

  • Develop a collaborative network of providers and resource organizations. This network will be a comprehensive resource for low-income adults with disabilities and their caregivers living throughout East King County to get support for essential services.

  • Identify the significant needs of low-income seniors, adults with disabilities, and their caregivers living in East King County, so we can focus our efforts on what matters most to those who need these critical services.

  • Strengthen partnerships with specialty organizations to create collaboration across providers and provide a client-first care plan.

  • Educate and connect low-income seniors, adults with disabilities, and their caregivers to our network of providers and resource organizations, based on the client’s individual needs.

  • Track and measure our successes and room for growth so we all can continue to improve and better serve low-income seniors, adults with disabilities, and their caregivers.
